Can you tell me how to cut off a T-shirt and add a fabric skirt to make a dress for a 4-year-old girl?

You can make these dresses for any age–the directions are the same.

Cut off the shirt just below the bustline, allowing an extra 1/2" for a seam allowance. (Measure about 2" from the lower armhole for a 4-year-old.)

Place the T-shirt on a flat surface and measure the width at the trimmed edge. For a skirt with full gathers, double the T-shirt width, and add 1" for seam allowances. For the length, measure from just below the bustline to the desired length, then add 1/2" for the upper seam allowance and 2" for the hem allowance. Cut two fabric rectangles with these dimensions.

With right sides together, stitch the skirt side seams. Stitch two rows of gathering stitches, 1/4" and 1/2" from the upper edge.

Gather and pin the skirt to fit the T-shirt, placing the skirt side seams at the underarms. Stitch the skirt to the T-shirt and hem the lower edge.
